Paternity Leave Application Form - SC3 Form

If you are employed by Hotel Chocolat and your partner is due to give birth or if you will be the biological

father of the baby, you will be entitled to take two weeks' paternity leave. To be eligible you must also have

responsibility for looking after the baby and be making the request to help care for the baby or to support

the baby's mother.

Please complete this form and send it to your Line Manager as soon as possible but no later than 15 weeks before your baby is due; to notify the business when you are expecting your baby, if you are going to take either one or two weeks off work, and when you would like your paternity leave to start. After you have submitted your SC3 form, your manager

will forward it to the hr@hotelchocolat.com and the Payroll Team will be able to confirm your

entitlements to Statutory Paternity Pay (SPP).
