Maternity Application Form

Submit your Maternity Application form along with your MatB1 no later than 15weeks before baby is due (at 25 weeks). 

The Maternity Application Form will ask your due date, when you would like your maternity leave to start, and whether you plan to return to work after maternity leave. 

Once this is submitted and processed you will receive a maternity confirmation letter from us. 

This will confirm your chosen maternity start date and based on this, your maternity end date and return to work date. 

This will be assumed as 52 weeks as this is the full maternity entitlement -  you do not at this stage have to disclose how long you plan to take off.
