You should have received the details with your onboarding papers but if you can't locate them, please fill in the form attached.
This is a taxable benefit, but the company will pay for your single cover.
If you want to enrol your family members, there will be a monthly deduction based on the cover type. Please see attached a Monthly Premium so you can see the costs associated with it.
